This form has been created as a way for First Nations to communicate to Indigenous Services Canada that they are ready to participate in consultations on the second-generation cut-off and section 10 voting thresholds.
Filling out this form is optional, but strongly encouraged. By completing this form, you will provide us with insights into your community's readiness for consultation, informing the timing and structure of the upcoming consultations. A response is appreciated by October 30th, 2024.

The collection, use and disclosure of personal information by Registration and Membership is authorized under the Indian Act, and is in accordance with the requirements of Privacy Act . Information collected will be used exclusively to assess First Nation's readiness for participation in the consultation process on the second-generation cut-off and section 10 voting thresholds. The collection, use and disclosure of your personal information (first and last name, email address, position title, First Nation, and Band number) is required for verifying your authority as a representative of the band. Personal information will be retained pursuant to the Privacy Act and its Regulations. Individuals have the right to the protection of, access to and request the correction of their personal information under the Privacy Act.
